I took my reg apart tonight. I had scoring on the big piston and scoring in the reg housing to match. Almost like what you might see on the poppit. I have never taken it apart. I polished it a little to take out some of the scoring. I had some inconsistantcy issues when adjusting the regulator. I eventually found the spot on the reg that was consistant and never touched it again (160psi).
Could this scoring been the source of the reg inconsistantcy?
I also noticed I have 2 black O-rings on my piston. In the pictures and schematics I see it looks like one of them should be urethane. Do you guys know what size they are?

Any metal to metal wear is not a good thing. Polishing out the gouges and finding an adjustment area not affected by the gouge is about all you can do - short of swapping the reg body.
As for the orings, I've seen Axes both ways: with and without the urethane oring. Urethane typically wears better and has a bit less drag (from my understanding). If the rubber oring is lubed regularly and replaced as wear occurs, I don't see it being a huge problem.
