Programming itself requires little to no math. Computer Science, however, requires a fair amount of math to do anything really useful.

Depends on what you want to write, basic stuff requires no more than basic algebra. More complex stuff like game engines (It's just an example) require heavy amounts of physics knowledge and calculus to write.
It really depends on what you're writing.

To be blunt, if you have to ask, you're not going to have an easy time making "anything that makes money"
Software development can be lucrative, but there is a serious amount of talent and thereby competition. You can't just cook something up without the proper knowledge, and practice.
Programming is first, and foremost problem solving. This requires a certain level of mathematical comprehension, not because it's heavy in math, but rather that the concepts used to solve these problems are akin to those that you use to solve math problems.

I would suggest looking at computer science curriculums. They will include several calculus and physics courses within the first two years, most of the programming will be in java as it teaches excellent programming practices. Also database design is typical since it'll teach you how to work with and manipulate sets of data.
Edit: I think MIT has free online courses for this type of stuff

I think what many programmers esp. those that haven't had formal education overlook the the architecture that is involved in programming/software development. Gone are the days of being a dedicated programmer or architect, you must be one in the same now days and plenty of companies are phasing out separate positions. As far as I know and have seen there is only one MIT free course online serving as a pilot course, and it was more on the electronics side of things, but I haven't looked at them in a while.

How hard is it to set up a really basic server and database for a web page assuming I have proficient programming skills but no networking/web systems skills?
