If everybody in your crew survives I don't see why they wouldn't be in the game. Playable, I'm not seeing anything official on Samara but I think Bioware is being a little tight lipped still until release.
She did, but I found it odd that Bioware confirmed numerous people to be in ME3, and did not for Samara only. It's like "why confirm any at all if you will confirm everyone cept 1 person?"

The only person I do not have, is Jack. I'm curious to see what Wrex does though. If I had to guess who has a dissapointing role, I would venture to guess Jacob, Jack and Thane. Complete guess though. One is dying, the other two never had a large fan base.
I'm not only curious to see how certain characters are involved in three as well as which few can join your squad, but what the game would be like if most of your crew dies, and how that plays out. See, the fact that a few people may of opted to go into ME3 with a bulk of their crew dead, leads me to think that most of the ME2 crew will not return due to that. I knowa guy who selected Jack to go into the tunnel and Jacob as his biotic to shield the crew, deliberately trying to get both of them killed. It worked, haha. He sent Zaeed to escort people back, but that one panned out, much to his regret. He hasn't tried to alter it since then.
