Yes, you can over tighten the fill nipple. It would bottom out and not allow the shuttle valve to open.
Make sure the tank is empty, remove the fill nipple, place some thread sealant on it and then screw it in hand tight. Use a wrench and give it one and a half to two full turns and that should set you straight. If you don't feel comfortable, have your field tech do it and try again.
If that doesn't do anything, then there is something else wrong with it.

As far as safety, all tanks have to meet DOT standards. Reliability however, is a totally different issue.

If you look under the fill nipple and there is a patent number, then we were the one's who made the regulator. If you tighten the fill nipple too much, it will bottom out the strut and not allow the tank to fill. You will need to loosen the fill nipple in order to get it to fill properly. When the tank is empty, you should be able to hear the strut rattle when you shake the tank.

As for safety, the regulator has all of the same safety features as the Ninja regulators. We pride ourselves in industry leading safety standards.
