How many teams actually have them?

I would say that it depends a lot on the format.
I know for my team we pretty much always have someone out there coaching, even if it is just a friend, but that is just because of the series and formats we play.

We have a bunch of local semi-pro and pro players helping us from time to time, when they can. Normally, we have some team/school alum coming out to coach us and occasionally, play.

The team I used to play for and who have now formed a new team are coached by Mike Paxson and used to be coached by a few of the guys from naughty dogs. I liked having experienced coaches to tell me what I'm doing wrong and what I should be doing to correct those things.

PSP Race2 format

If the other team is coaching, we will coach. I don't personally believe in it but you certainly can't let your team possibly lose because the other team is getting coached. There is no designated coach but it's most always me. It's harder for some because they only play a position or two and don't know the mechanics of how it all works in every position. I won't coach a 1 v 1 though other than tips about playing (rather than where the other player is or what he is doing)

That's for in-game coaching. I also draw up all the breakouts, strategies, assign jobs, etc. as far as what you might consider off the field coaching
